Basic Move and Contact Information
Please provide the name and address for the person that you would like to authorize to act on your behalf at delivery.
You may find your order number on your order for service. It is a 6 digit number following the letter "L".
Long distance moves are ONLY if your delivery has a delivery window. If you already have a confirmed delivery date, you may select local even if your move is crossing state lines.

Any restrictions not noted that will delay or interfere with your move may result in additional fees.

Delivery Address Confirmation
Long Distance Delivery Information
Your First Available Date for delivery is defined as the first full day that you have possession of the delivery address and an authorized party will be present and able to receive delivery. We require 2-4 business days notice to start your delivery window. Your Fist Available Date cannot be sooner than the day after your pickup date.
**Changes to the delivery window while in transit will result in re-delivery costs as well as additional storage cost. Client MUST be available throughout the entire delivery window.
All long distance deliveries are completed with a tractor-trailer. In the event there is no access for the tractor-trailer at delivery, White Glove will need to rent a "Shuttle Truck" and an additional fee will apply as per your Order For Service.
The final determination of access for a tractor-trailer is made by the driver upon arrival. The driver will assess the location to ensure your delivery can be completed safely.
All long distance deliveries are completed with a delivery window. Your delivery window starts on the First Available Date selected above. Delivery can occur as early as your First Available Date and up to and within the number of business days indicated in your Order For Service. Please confirm your delivery window below.
Local Delivery Date Request
Requesting a delivery date does not guarantee date. Deliveries are ALWAYS subject to availability. White Glove requests at least 72 business hours notice of a requested delivery. Once this form is completed, an associate will reach out to you to confirm availability and pricing for your requested date.
